Therapist - Physical Therapist

Location: San Jose, CA

Schedule / Job Type: Full-time, travel, 13-week assignment, days shifts, typically 4 days per week

Key Responsibilities:

• Provide skilled physical therapy services to patients in an acute care or skilled nursing facility setting
• Perform physical therapy evaluations and treatments in accordance with physician orders and standards of care
• Document patient progress and maintain accurate records using Meditech EMR system
• Collaborate with a multidisciplinary team to develop and implement patient care plans
• Work comfortably in a fast-paced environment, ensuring quality care

Required Qualifications:

• Valid California Physical Therapist license
• Minimum of 2 years of experience in acute care or skilled nursing settings
• Ability to perform PT evaluations and treatments effectively
• Flexibility to work 8 or 10-hour shifts, including weekends as needed

Preferred Experience / Skills:

• Experience in hospital or acute care environments
• Strong communication and teamwork skills
• Ability to adapt quickly in a dynamic setting
