Key Responsibilities
- Provide skilled home health nursing care to patients in their residence.
- Manage and coordinate patient care plans in collaboration with healthcare teams.
- Perform OASIS assessments and ensure compliance with clinical documentation standards.
- Educate patients and families on health management and recovery processes.
- Document patient progress and communicate effectively with interdisciplinary teams.
- Ensure safety and quality standards are upheld during all care activities.
Required Qualifications
- Valid Registered Nurse (RN) license in the state of Vermont.
- Home health and case management experience.
- Experience with OASIS assessments.
- Strong clinical skills with a focus on home healthcare.
- Ability to travel between patient locations within the service area.
- Reliability in availability with no requested time off for the contract duration.
Preferred Experience / Skills
- Prior travel nursing experience.
- Excellent communication and organizational skills.
