Schedule / Job Type

• Days, part-time (4 shifts per week), 10-hour shifts, home health setting

Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability
• Develop and implement treatment plans aimed at helping clients achieve their maximum performance within their capabilities
• Document observations, clinical findings, interventions, goals, and discharge plans accurately and timely
• Supervise Physical Therapy Assistants and perform initial admission and evaluation visits as needed
• Notify the Clinical Team Manager of OASIS scores within established timelines
• Perform additional duties aligned with skills and the organization’s goals in the home health setting

Required Qualifications

• One year of home health physical therapy experience
• Valid state licensure as a Physical Therapist

Preferred Experience / Skills

• Experience working in multiple territories including Milpitas, Saratoga, Los Gatos, and surrounding cities
• Ability to work independently and collaborate with a multidisciplinary team
