Key Responsibilities

• Provide high-quality physical therapy evaluations and treatments for residents.
• Develop and implement individualized therapy plans to improve mobility, safety, and overall quality of life.
• Collaborate with rehab team, nursing staff, and other healthcare partners.
• Document services accurately using applicable electronic health record systems.
• Support a positive, resident-centered culture within the therapy department.

Required Qualifications

• Pennsylvania Physical Therapist license in good standing.
• Strong communication and teamwork skills.
• Experience working in senior care settings.
• CPR certification through The American Heart Association or The Red Cross.
• Two-step PPD or TB skin test within 30 days of start date.
• Covid vaccination records or valid exemption.
• Background check and fingerprinting completed.
• Physical performed by a physician, physician assistant, or nurse practitioner.

Preferred Experience / Skills

• Prior experience working with older adults in skilled nursing or assisted living environments.
• Familiarity with electronic health record systems such as Net Health.
