Registered Nurse - Home Health
Location: Lacey, WA
Schedule / Job Type: Full-time, Day shift, 5 days per week, 8-hour shifts
Key Responsibilities:

• Provide home health nursing care to geriatric patients within a designated service territory.
• Administer treatments, monitor patient health, and document care accurately and timely.
• Coordinate with interdisciplinary teams to ensure quality patient outcomes.
• Complete required documentation using HCHB EMR system, including OASIS assessments for SOC.
• Maintain compliance with all safety, documentation, and communication standards.
  Required Qualifications:
• Active RN license in the state of Washington.
• Minimum of 1 year of recent relevant home health experience.
• Experience with HCHB EMR system and OASIS assessments required.
• BLS certification required.
• Valid driver’s license and access to personal transportation.
  Preferred Experience / Skills:
• Proficiency in HCHB (Homecare Homebase) platform.
• Experience with hospice or SOC (Start of Care) assessments.
• Flexibility to cover a territory within a 60-mile radius and adapt to travel requirements.
