- Full-time, Days shifts, 5 shifts per week, 8 hours per shift
Key Responsibilities
- Provide end-of-life hospice and geriatric care to patients within assigned territory
- Document patient care and progress accurately, ensuring completion by the following morning
- Communicate effectively with the care team and family members regarding patient status
- Coordinate and manage patient visits within a 60-mile radius of the office
- Participate in on-call rotation as required (weekend coverage may be needed)
Required Qualifications
- Valid and current RN license in Washington state
- Minimum of 1 year of relevant hospice or home health experience
- Proficiency in HCHB (Homecare Homebase) preferred
- BLS certification required
- Must have an active driver’s license and reliable transportation
- Experience with OASIS for SOC (Start of Care) in home health is required
Preferred Experience / Skills
- Recent experience working in hospice or geriatric end-of-life care
- Familiarity with documentation systems and timely communication protocols
- Ability to handle a fast-paced environment with multiple patient visits daily
