Job Title

• Registered Nurse - Hospice

Schedule / Job Type

• Full-time, Days shifts, 5 shifts per week, 8 hours per shift

Key Responsibilities

• Provide end-of-life hospice and geriatric care to patients within assigned territory
• Document patient care and progress accurately, ensuring completion by the following morning
• Communicate effectively with the care team and family members regarding patient status
• Coordinate and manage patient visits within a 60-mile radius of the office
• Participate in on-call rotation as required (weekend coverage may be needed)

Required Qualifications

• Valid and current RN license in Washington state
• Minimum of 1 year of relevant hospice or home health experience
• Proficiency in HCHB (Homecare Homebase) preferred
• BLS certification required
• Must have an active driver’s license and reliable transportation
• Experience with OASIS for SOC (Start of Care) in home health is required

Preferred Experience / Skills

• Recent experience working in hospice or geriatric end-of-life care
• Familiarity with documentation systems and timely communication protocols
• Ability to handle a fast-paced environment with multiple patient visits daily
