Key Responsibilities
- Provide comprehensive nursing care to hospice patients in a home health setting
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Monitor patient conditions and adjust treatments as necessary
- Educate patients and families on disease management and care procedures
- Document patient assessments, interventions, and progress in compliance with healthcare regulations
Required Qualifications
- Active RN license in Tennessee or valid compact license
- Minimum of 2 years of nursing experience, preferably with hospice or home health care
- Strong clinical assessment and communication skills
- Ability to work independently and manage a caseload effectively
Preferred Experience / Skills
- Experience in hospice or palliative care settings
- Knowledge of Cerner or similar electronic medical record systems
- Experience with reaching and maintaining compliance with travel and licensure requirements
