Schedule / Job Type

• Night shifts, 3 shifts per week, long-term care / skilled nursing facility

Key Responsibilities

• Provide quality nursing care to long-term patients in a skilled nursing environment.
• Assist with daily patient assessments and documentation, primarily using EPIC system.
• Support patient safety and comfort, adhering to established protocols.
• Collaborate with healthcare team members to meet patient needs and ensure continuity of care.
• Float between units as needed, with occasional participation in specialty testing or specific unit requirements.

Required Qualifications

• Valid LVN / LPN license in the state of Vermont.
• Minimum of 1 year of licensed clinical experience.
• Experience working in LTC or SNF settings preferred.
• EPIC EMR experience from within the past 2 years is required.
• Ability to work every other weekend and holidays.
• Possession of or willingness to obtain dementia capable care certification is preferred.

Preferred Experience / Skills

• Prior travel nursing experience.
• Experience with long-term patient populations aged 60+.
• Comfort working in a fast-paced, variable schedule environment.
