Key Responsibilities

• Provide compassionate, safe, and efficient nursing care to residents in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting.
• Assess, plan, implement, and coordinate individualized resident care plans in accordance with facility standards and regulations.
• Administer medications and treatments accurately and document care provided using electronic medical records (EMR).
• Monitor residents’ conditions, report significant changes, and update care plans as needed.
• Safeguard residents’ property, privacy, and confidentiality, promoting independence and dignity.
• Assist with transfers and mobility using appropriate equipment, ensuring residents feel safe during assistance.
• Respond promptly to resident call lights and assist with daily activities, ensuring a safe and pleasant environment.
• Educate residents and families about care plans and health concerns, providing excellent communication and support.
• Follow all safety protocols, including fire and disaster procedures, and maintain a clean, orderly living environment.

Required Qualifications

• Graduate of an accredited nursing program.
• Valid RN licensure for the state of practice.
• Basic Life Support (BLS) certification.
• Strong communication skills in English (oral and written).
• Knowledge of basic nursing skills and medication administration.

Preferred Experience / Skills

• Previous experience in LTC/SNF settings preferred.
• Familiarity with electronic health records (EHR/EMR) systems.
• Ability to operate transfer equipment such as gait belts, lifts, and slide boards.
