Schedule / Job Type

• Full-time, day shifts, 5 days per week

Key Responsibilities

• Provide home hospice nursing care to patients in accordance with care plans
• Case manage and coordinate patient needs, ensuring quality and compliance
• Document patient care activities accurately in electronic health records
• Travel throughout the service area using own transportation to reach patient locations
• Collaborate with the interdisciplinary team to support patient comfort and safety

Required Qualifications

• One or more years of hospice or home care nursing experience
• Valid nursing license (temp or pending licenses accepted, with confirmation of license prior to start)
• Willingness to travel extensively, with own vehicle and mileage reimbursement provided at Fed rate
• Ability to work independently and manage a caseload effectively
• Experience with electronic charting systems (HomeCare HomeBase preferred)
• Must comply with travel policies and be capable of working some holidays

Preferred Experience / Skills

• Prior travel or home health experience
• Certification in hospice or palliative care preferred (additional certifications such as ARC accepted)
• Ability to complete required modules during orientation within 30 days of start
