Job Title

Key Responsibilities
• Perform discharge planning, initial evaluations, and assessments for hospital patients using CarePort Care Management and Meditech EMR systems
• Coordinate referrals for post-acute resources such as home health, DME, home infusion, hospice, SNFs, and long-term care facilities
• Support interdisciplinary team efforts, including arranging discharge transportation and reassessment of patient needs
• Assist with documentation, referral submission, and resource management within the hospital's case management department
• Collaborate with hospital staff across various units, including ER, ICU, NICU, PCU, and Med-Surg, adhering to hospital policies and protocols

Required Qualifications
• Minimum of 2 years of case management experience in an acute hospital setting
• Proficiency with Meditech EMR and CarePort Care Management systems
• Valid VA license or Compact license at time of submission
• BLS and ACLS certifications are required
• Experience with arranging post-acute services and resource management

Preferred Experience / Skills
• Prior hospital-based social work or case management experience
• Certification as ACMA or CCM is preferred but not required
