Full-time, 5 days per week, Day shift (8:00 AM - 5:00 PM)
Key Responsibilities
- Manage a caseload of home health patients within the Greensboro/High Point area.
- Provide comprehensive nursing care and patient education in a home setting.
- Document patient interactions accurately using HomeCare HomeBase or similar charting systems.
- Travel throughout the day to various patient locations, requiring the use of personal vehicle.
- Ensure compliance with travel policies and maintain required certifications.
- Collaborate with healthcare teams to deliver quality patient care.
Required Qualifications
- Minimum 1 year of home health or case management experience.
- Valid or pending RN license; confirmation of licensure prior to start preferred.
- Ability to travel extensively within the service area.
- Willingness to work some holidays.
- Reliable transportation and current auto insurance for mileage reimbursement.
- Minimum of two supervisory references.
Preferred Experience / Skills
- Prior experience with HomeCare HomeBase or similar charting software.
- Knowledge of home health regulations and compliance policies.
- Experience with travel policies and documentation processes.
