Job Title

Schedule / Job Type

• Rotating shifts, 1 shift per week

Key Responsibilities

• Perform diagnostic assessments and develop individualized treatment plans for patients
• Deliver professional physical therapy services within the scope of practice
• Work primarily in outpatient settings, with potential assignments in skilled nursing facilities and inpatient units
• Utilize EMR systems to document patient care and progress
• Contribute to a multidisciplinary team to support patient habilitation and rehabilitation needs
• Adhere to all hospital policies and provide flexible coverage, including weekends and holidays as needed

Required Qualifications

• Hawaii state license in physical therapy (must obtain prior to start; active license required at start date)
• BLS certification
• Minimum of 6 months of work experience in adult care settings such as acute care, SNF, rehab hospital, or outpatient

Preferred Experience / Skills

• Experience working in outpatient, inpatient, or SNF environments
• Familiarity with Meditech EMR system
• Ability to work flexible shifts, including weekends and holidays
