Key Responsibilities

• Provide social work therapy and support to clients in a home health setting.
• Collaborate with healthcare teams to develop and implement patient care plans.
• Document patient progress and maintain accurate records in compliance with agency standards.
• Assist patients and families with resource planning and emotional support.
• Ensure timely communication and proper documentation of all client interactions.
• Adhere to agency policies and safety protocols during home visits.

Required Qualifications

• Valid social work license or certification in the state of Washington.
• Minimum of 2 years of experience in home health therapy or social work.
• Proficiency in documentation systems such as HCHB (Homecare Homebase).
• Experience with OASIS data collection and SOC processes preferred.
• BCLS certification required.
• Valid driver’s license and reliable personal transportation.

Preferred Experience / Skills

• Experience providing services in a home health or hospice environment.
• Knowledge of Medicare and Medicaid regulations related to home health.
• Strong communication and documentation skills.
