Key Responsibilities
- Provide social work services to clients in a home health setting
- Collaborate with healthcare teams to develop and implement care plans
- Document patient interactions and progress accurately and timely
- Ensure compliance with all regulatory and agency guidelines
- Assist clients with resource coordination and emotional support
Required Qualifications
- Active State license as a Social Worker (LSW or similar)
- Minimum of 2 years of experience in skilled home health care
- Proficiency with HCHB (Homecare Homebase) system
- Experience with OASIS assessments required for SOC (Start of Care) visits
- BCLS certification
- Valid driver’s license with personal transportation
Preferred Experience / Skills
- Prior hospice or home health experience preferred
- Experience working with diverse patient populations
