Key Responsibilities
- Partner with residents and care teams to deliver physical therapy aimed at maximizing residents’ functional abilities and independence.
- Conduct evaluations and develop personalized treatment plans including short- and long-term goals, therapy modalities, and care frequency.
- Collaborate with residents, caregivers, and healthcare team members to ensure comprehensive, resident-centered care.
- Monitor progress, adjust treatment plans as necessary, and document all interactions accurately in the Electronic Medical Record.
- Educate residents and caregivers on assistive devices and therapeutic exercises, ensuring proper use and understanding.
- Communicate with physicians and team members about treatment plans and discharge needs.
- Supervise and mentor assistant-level staff, ensuring adherence to professional standards and regulations.
- Maintain department cleanliness, equipment, and compliance with infection control procedures.
- Stay current with professional development through memberships, publications, and in-service education.
- Assist in developing new programs and services within the rehabilitation department.
Required Qualifications
- Active license as a Physical Therapist in the applicable state.
- Bachelor’s degree in a related field; Master’s or Doctorate preferred.
- Basic Life Support (BLS) certification preferred.
- Prior experience in long-term care or skilled nursing facility preferred.
- Strong communication, documentation, and interpersonal skills.
- Proficiency in Microsoft Office.
Preferred Experience / Skills
- Experience working in elder care or rehabilitation settings.
- Knowledge of Medicare, Medicaid, and private insurance reimbursement regulations.
- Ability to work collaboratively within an interdisciplinary team.
