Key Responsibilities

• Partner with residents and care teams to deliver physical therapy aimed at maximizing residents’ functional abilities and independence.
• Conduct evaluations and develop personalized treatment plans including short- and long-term goals, therapy modalities, and care frequency.
• Collaborate with residents, caregivers, and healthcare team members to ensure comprehensive, resident-centered care.
• Monitor progress, adjust treatment plans as necessary, and document all interactions accurately in the Electronic Medical Record.
• Educate residents and caregivers on assistive devices and therapeutic exercises, ensuring proper use and understanding.
• Communicate with physicians and team members about treatment plans and discharge needs.
• Supervise and mentor assistant-level staff, ensuring adherence to professional standards and regulations.
• Maintain department cleanliness, equipment, and compliance with infection control procedures.
• Stay current with professional development through memberships, publications, and in-service education.
• Assist in developing new programs and services within the rehabilitation department.

Required Qualifications

• Active license as a Physical Therapist in the applicable state.
• Bachelor’s degree in a related field; Master’s or Doctorate preferred.
• Basic Life Support (BLS) certification preferred.
• Prior experience in long-term care or skilled nursing facility preferred.
• Strong communication, documentation, and interpersonal skills.
• Proficiency in Microsoft Office.

Preferred Experience / Skills

• Experience working in elder care or rehabilitation settings.
• Knowledge of Medicare, Medicaid, and private insurance reimbursement regulations.
• Ability to work collaboratively within an interdisciplinary team.
