We are seeking a compassionate and skilled Physical Therapist to join our team at Ridgewood Center, a leading skilled nursing facility dedicated to providing exceptional care and rehabilitation services to individuals recovering from illness or injury.

About the Role:

As a Physical Therapist at Ridgewood Center, you will play a vital role in helping our patients achieve their rehabilitation goals and regain their independence. You will work closely with our multidisciplinary team to develop and implement personalized treatment plans, conduct assessments, and provide evidence-based care to patients.

Responsibilities:

• Conduct comprehensive assessments of patients to identify rehabilitation needs and develop individualized treatment plans • Develop, implement, and progress patients through various physical therapy interventions, including exercise, manual therapy, and modalities • Collaborate with the interdisciplinary team to coordinate patient care and ensure seamless communication • Educate patients and their families on proper exercise techniques, lifestyle modifications, and home safety measures • Document patient progress and outcomes in accordance with regulatory requirements • Participate in case conferences and contribute to quality improvement initiatives

Requirements:

• Doctoral degree in Physical Therapy (DPT) from an accredited program • Current licensure as a Physical Therapist in the state of State • 1+ year of experience in inpatient or outpatient physical therapy settings • Strong clinical skills and ability to work effectively with patients with diverse needs and abilities • Excellent communication and interpersonal skills • Ability to work collaboratively as part of a multidisciplinary team

What We Offer:
