Registered Nurse - Nurse Care Coordinator

Key Responsibilities:
- Manage the active care of patients within the organization, collaborating with medical providers and interdisciplinary teams to develop individualized treatment plans aimed at optimizing health outcomes.
- Coordinate multi-disciplinary care conferences for high-risk or complex patients and facilitate communication among care team members.
- Monitor patient panels for acute and chronic conditions, providing outreach, education, and health maintenance based on EHR prompts and triggers.
- Serve as a primary point of contact for referrals, tracking progress, facilitating timely follow-up, and ensuring documentation is complete and accessible.
- Perform staff nursing duties including patient assessment, documentation, patient education, and administration of medications or therapies.
- Support specialty clinic processes by coordinating referrals, preparing clinical spaces, assisting with procedures, and ensuring diagnostic tests are scheduled and completed.
Required Qualifications:
- Active and unrestricted Registered Nursing license in Alaska at the time of application.
- Minimum of 2 years recent clinical nursing experience, including at least 6 months in a healthcare setting such as an outpatient clinic or ambulatory care.
- Proven experience in case management or care coordination.
- Ability to work independently in a fast-paced clinical environment.
- Strong assessment, communication, and organizational skills.
- Basic Life Support (BLS) certification.
Preferred Experience / Skills:
- Experience working within tribal health organizations or similar healthcare systems in Alaska.
- Knowledge of electronic health records (EHR) systems.
- National Case Management Certification.
- Experience with specialty care coordination, OB care management, or telehealth services.
