Key Responsibilities
• Provide physical therapy assessments and treatments to patients
• Develop and implement individualized care plans
• Collaborate with healthcare teams to ensure optimal patient outcomes
• Document patient progress and maintain accurate records
• Prefer candidates with CCRC (Continuing Care Retirement Community) experience
Required Qualifications
• Valid state license as a Physical Therapist
• Proven experience in delivering physical therapy services
• Strong patient assessment and care planning skills
Preferred Experience / Skills
• Experience working in a CCRC setting
• Excellent communication and teamwork skills
