Schedule / Job Type
- Day shift, Full-time, Monday through Friday, no weekend rotation
Key Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility setting
- Develop and implement individualized treatment plans based on patient needs
- Monitor patient progress and adjust treatments accordingly
- Collaborate with healthcare team to ensure optimal patient outcomes
- Document patient care activities accurately and timely
Required Qualifications
- Valid Physical Therapist license or credential
- Prior experience in a skilled nursing or similar facility preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Preferred Experience / Skills
- Experience working in a SNF setting
- Knowledge of relevant healthcare regulations and documentation standards
