Key Responsibilities

• Provide high-quality physical therapy evaluations and treatments for residents.
• Develop and implement individualized therapy plans to improve mobility, safety, and quality of life.
• Collaborate with rehab team, nursing staff, and other healthcare providers.
• Document services accurately using Net Health (training provided).
• Support a positive, resident-centered culture within the therapy department.

Required Qualifications

• PA Physical Therapist license in good standing.
• CPR certification through the American Heart Association or Red Cross.
• Two-step PPD or TB skin test within 30 days of start date.
• COVID vaccination records or exemption.
• Background check and fingerprinting.
• Physical performed by a physician, physician assistant, or nurse practitioner.
