Key Responsibilities
- Provide direct nursing care to residents in an LTAC setting, ensuring timely and accurate assessments.
- Communicate significant changes in resident conditions to physicians and families promptly.
- Administer medications following the 5 Rights and perform wound care as needed.
- Assist Certified Nursing Assistants with resident needs and respond to call lights.
- Monitor residents for changes in condition and act as a floor leader to assess and manage situations.
- Maintain detailed and accurate resident records and documentation.
- Report any allegations of abuse, neglect, or misappropriation to supervisory staff.
- Perform additional duties as assigned to support resident care and facility operations.
Required Qualifications
- Two-year degree in nursing (associate degree) required; four-year degree preferred.
- Valid Wisconsin Registered Nursing license and ongoing licensure maintenance.
- Current CPR/BLS certification.
- Exceptional attention to detail and accuracy in clinical documentation.
- 1+ years of nursing experience in a healthcare setting preferred.
Preferred Experience / Skills
- Experience working in long-term acute care (LTAC) or similar environments.
- Strong communication skills and ability to work effectively in a team.
