Key Responsibilities
• Provide physical therapy services to patients according to prescribed treatment plans
• Complete documentation and communicate directly with the employer’s credentialing team prior to start
• Ensure compliance with licensing and facility requirements
• Assist with equipment setup during interviews and onsite visits
• Maintain accurate patient records and progress reports

Required Qualifications
• Active state license as a Physical Therapist
• Experience in a clinical setting, specifically in setting-based therapy
• Ability to complete documentation and coordinate with credentialing teams
• Available to work full days (8 hours shifts)
• Must be able to start on 12/22

Preferred Experience / Skills
• Prior experience working with outpatient or hospital patients
• Strong communication skills and teamwork orientation

Additional Details
• Shifts: 8 hours per day, days only
• Duration: Contract with weekly billing cycle
• Notice Period: 30 days
• Parking and dress code to be provided on the first day
• Holiday weeks will have 32 hours included
• OT is paid at 1.5x over scheduled hours
• Starting pay rate: $80/hour (bill GWW 40)
• Billing cycle: Weekly
