Key Responsibilities
• Provide skilled physical therapy services to residents in a licensed skilled nursing facility
• Develop and implement individualized treatment plans
• Assess patient progress and adjust therapy plans accordingly
• Collaborate with healthcare team to ensure quality patient care
• Document treatment sessions and patient outcomes
• Maintain compliance with all licensing and facility regulations

Required Qualifications
• Valid state licensure as a Physical Therapist
• Current state license or registration as required
• Strong understanding of physical therapy principles and techniques
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Preferred Experience / Skills
• Prior experience in a skilled nursing or long-term care setting
• Knowledge of Medicare/Medicaid regulations and documentation
• Bilingual abilities a plus
