Summary

iDEAL HIRE is hiring a new Occupational Therapist - Skilled Nursing / Rehab for our client, a modern 54-bed Skilled Nursing Facility. This is a Permanent role, working in Phoenix, AZ, offering a starting pay rate of $55,000.

What You'll Be Doing

  • Patient Assessment: Evaluate referred patients to determine medical condition, functional capabilities, and rehabilitation potential.

  • Treatment Planning: Establish and administer customized treatment programs with specific goals based on patient capacity and physician direction.

  • Clinical Supervision: Provide consultation and clinical supervision to Certified Occupational Therapy Assistants (COTAs) and Rehab Aides within the facility.

  • Interdisciplinary Collaboration: Actively integrate therapy’s role into the facility’s interdisciplinary treatment team and participate in family conferences and case management meetings.

  • Documentation: Maintain thorough records of patient progress and adhere to Minimum Data Set (MDS) procedures and effective care planning.

  • Resource Management: Direct staff in skilled tasks during group and individual treatments to ensure efficient utilization of resources.

  • Education: Instruct patients and their families in appropriate occupational therapy methods and home safety techniques.


What We're Looking For

  • Education: Must have graduated from an accredited school of Occupational Therapy.

  • Licensure: Current state license as an Occupational Therapist in Arizona.

  • Certification: Current CPR certification required.

  • Experience: Six months of experience in a long-term care (LTC) or skilled nursing environment is preferred.

  • Technology: Familiarity with Net Health and PCC EMR systems is a plus.

  • Soft Skills: Strong interpersonal excellence and a commitment to person-centered care within a collaborative, modern facility culture.
