Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Float to Camp Verde as needed to ensure patient care coverage.
• Deliver therapy sessions with an 8-hour shift structure, maintaining compliance with facility guidelines.
• Collaborate with healthcare team to develop and implement patient treatment plans.
• Document patient progress and response to therapy interventions accurately.
• Ensure licensing requirements are met and maintained according to state regulations.

Required Qualifications

• Valid Arizona state license for physical therapy upon submission or proof of application with an ETA for licensing approval.
• Experience working in a skilled nursing facility (SNF) preferred.
• Ability to float between locations as needed.
• Good documentation and communication skills.
• Flexibility to work in a fast-paced healthcare environment.

Preferred Experience / Skills

• Prior experience in SNF or long-term care settings.
• Strong understanding of patient treatment planning and documentation.
• Knowledge of state and federal healthcare regulations related to therapy services.
