Full-time, Day shifts, Monday through Friday, 8-hour shifts
Key Responsibilities
- Provide physical therapy services to patients in a home health setting
- Develop and implement personalized treatment plans to improve patient outcomes
- Monitor patient progress and modify treatment as necessary
- Educate patients and caregivers on exercises and therapy techniques
- Maintain accurate documentation of treatment sessions and progress reports
Required Qualifications
- Valid state license as a Physical Therapist
- Ability to work independently in a home health environment
- Strong communication and patient care skills
- Flexibility to work typical weekday daytime hours
- Reliable transportation and ability to travel to patient locations
Preferred Experience / Skills
- Prior experience in home health or outpatient therapy is a plus
- Familiarity with pediatric or geriatric populations (if applicable)
- CPR/BLS certification
