Key Responsibilities

• Provide hospice nursing care in accordance with patient needs and care plans.
• Complete home health assessments and document using Homecare Homebase/OASIS systems.
• Manage wound care and utilize Wellsky EMR as needed.
• Participate in on-call triage for patient emergencies during designated hours.
• Collaborate with interdisciplinary team to ensure compassionate patient support.
• Maintain accurate and timely documentation of care provided.

Required Qualifications

• At least 2 years of hospice RN experience.
• Recent experience (within the last year) with Homecare Homebase/OASIS.
• Valid licensure as a registered nurse.
• Ability to handle on-call responsibilities from 5:30 PM to 8:30 AM, one to two weeks per month.
• Reliable, dependable, and excellent time management skills.
• Professional demeanor with a focus on customer service and compassion.

Preferred Experience / Skills

• Experience with Wellsky EMR system.
• Wound care experience.
• Experience with triage and phone-based patient assessments.
