Key Responsibilities

• Provide direct and indirect patient care following the nursing process of Assessment, Planning, Intervention, and Evaluation.
• Develop and update nursing care plans in coordination with patients, families, and interdisciplinary team members.
• Communicate changes in patient’s clinical condition to physicians, supervisors, and team members.
• Assist with discharge planning and ensure smooth transition of care.
• Perform patient assessments upon admission and during reassessments, documenting thoroughly in medical records.
• Administer medications accurately, following the five rights of medication administration.
• Assist physicians with examinations, treatments, and procedures within scope of practice.
• Educate patients and families on care plans, pain management, and health education, addressing psychosocial and cultural needs.
• Maintain high standards of nursing care according to hospital and departmental policies.
• Supervise and evaluate nursing team members, assigning care based on patient needs and team member capabilities.
• Serve as a liaison between administration, healthcare providers, patients, and families.

Required Qualifications

• Current state licensure as a Registered Nurse.
• BCLS certification.
• Minimum of six months’ Medical/Surgical experience in an acute care setting preferred.

Preferred Experience / Skills

• ACLS certification preferred.
• Experience working in a Long-Term Acute Care (LTAC) setting is a plus.
• Strong communication, organizational, and interpersonal skills.
• Basic computer skills for documentation and communication.
