Full-time, night shifts, 12-hour shifts, on-site
Key Responsibilities
- Provide direct patient care to residents in a long-term care or skilled nursing facility
- Assist with medication administration and patient assessments
- Support patients with activities of daily living and transfers
- Document patient care accurately in electronic medical records
- Collaborate with healthcare team to ensure quality care delivery
Required Qualifications
- Valid LVN or LPN license
- Experience working in a long-term care or skilled nursing environment
- Ability to work night shifts effectively
- Knowledge of common diagnoses such as hypertension and diabetes
- Familiarity with EMR systems, preferably PCC
Preferred Experience / Skills
- Prior experience with patient care involving ADLs and transfers
- Ability to work independently and as part of a team
- Strong communication and organizational skills
