Job Title

Schedule / Job Type
• Full-time, Day shifts (8 hours per day, 5 days per week)

Key Responsibilities
• Provide skilled outpatient physical therapy services to patients in a home health setting within Medford and surrounding areas
• Assess patient needs, develop treatment plans, and implement therapeutic interventions
• Document patient progress and maintain accurate records using EPIC/Oasis systems
• Collaborate with team members to ensure quality care and adherence to clinical protocols
• Maintain professional compliance with all licensing and facility requirements

Required Qualifications
• Valid Physical Therapist license for Oregon
• Minimum of 1 year of skilled home health experience
• BCLS certification
• Own transportation required; ability to visit multiple patients within a reasonable radius
• COVID-19 vaccination series, including one booster

Preferred Experience / Skills
• Experience using Oasis documentation systems
• Ability to work independently in a home health environment
• Strong organizational and time management skills

Additional Information
• Shift: 8:00 AM – 4:30 PM, 5 days per week
• Candidates considered "local" within 50-75 miles will be prioritized; local candidates will be considered at the same rate
• Guaranteed hours with up to one call-off per pay period
• This facility cannot accommodate 4x10s or 3x12s schedules
• Must be willing to float within the same facility network as needed
• COVID vaccination and booster required; travel compliance to all regulations

Travel and Certification Details
• All travelers must have reliable transportation and relevant Oasis experience if applicable
• References: Minimum of two supervisory references covering at least 1 year of employment in a comparable setting within the last three years
