Key Responsibilities
- Provide patient care in a Long-Term Care / Skilled Nursing Facility setting
- Work 12-hour day shifts from 6:00 AM to 6:00 PM
- Rotate weekends (every Saturday, Sunday, and Monday) as scheduled
- Support patient needs according to established protocols and guidelines
- Collaborate with healthcare team to ensure quality care delivery
Required Qualifications
- Valid LVN or LPN license
- Proven experience in LTC or SNF environments
- Ability to work 12-hour day shifts and weekend rotations
- Strong communication and teamwork skills
Preferred Experience / Skills
- Prior experience with weekend shift coverage preferred
- Familiarity with charting and patient record systems
