Key Responsibilities
• Provide physical therapy services to patients according to treatment plans.
• Complete documentation and progress notes in compliance with facility standards.
• Collaborate with healthcare teams to develop and implement patient care plans.
• Ensure compliance with licensing and credentialing requirements.
• Use provided equipment during treatment sessions.

Required Qualifications
• Active and valid state license as a Physical Therapist.
• Experience in setting and delivering physical therapy services.
• Ability to complete documentation accurately and timely.
• Availability to work 8-hour shifts during daytime hours.
• Willingness to complete required credentialing and documentation with the facility.

Preferred Experience / Skills
• Prior experience working in a hospital or outpatient setting.
• Knowledge of current physical therapy practices and equipment.
