Schedule / Job Type
- Full-time, night shifts, 8.5 hours per shift, 6 shifts per week
Key Responsibilities
- Provide quality care to residents in long-term care or skilled nursing facilities
- Assist with patient assessments, treatments, and medication administration
- Collaborate with healthcare teams to develop and implement patient care plans
- Document care provided accurately and timely
- Ensure compliance with all facility policies and healthcare regulations
Required Qualifications
- Valid LVN or LPN license in the state of New Hampshire
- Proven experience in long-term care or skilled nursing facilities
- Strong patient care and communication skills
- Ability to work effectively in a night shift schedule
- Knowledge of healthcare safety standards and protocols
Preferred Experience / Skills
- Previous experience working in LTAC or SNF settings
- Bilingual skills or additional language proficiency
- CPR and First Aid certification
