Key Responsibilities
- Provide nursing care to residents in a long-term care or skilled nursing facility
- Assist with patient assessments and medication administration
- Collaborate with healthcare team to develop and implement care plans
- Document patient care activities accurately and thoroughly
- Ensure compliance with facility policies and healthcare regulations
Required Qualifications
- Valid and active LVN or LPN license
- Minimum of 1 year of experience in long-term care or skilled nursing facilities
- Knowledge of electronic health records and medication administration procedures
- Ability to work independently and as part of a healthcare team
Preferred Experience / Skills
- Experience with elderly or chronically ill patients
- Strong communication and interpersonal skills
