Key Responsibilities

• Provide skilled nursing care to patients in their home environment in accordance with physician orders and care plans.
• Perform clinical visits including wound care, IV therapy (antibiotics, TPN), post-surgical assessments, and patient education.
• Utilize point-of-care tools such as wound vacs, glucometers, pro-time machines, IV pumps, and central lines.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care within assigned territories covering approximately 40-80 miles.
• Document all patient interactions and clinical findings accurately using the EPIC charting system.
• Float within scope to other PODs as needed, ensuring flexibility to meet facility requirements.

Required Qualifications

• Valid State Nursing License (MO/IL/Compact licensure preferred) at the time of submission.
• Minimum of 1-2 years of MS or acute care experience, with at least 1 year in home health care.
• Ortho experience required.
• BLS certification required.
• Phlebotomy certification preferred.
• Must be comfortable working day shifts, approximately 8 hours per shift, Monday through Friday.
• Ability to work every 3rd to 4th weekend as needed.
• Valid SSN and DOB required for onboarding.
• Ability to complete pediatric modules and comply with travel and compliance requirements.

Preferred Experience / Skills

• Experience in Wound Vac, IV therapy, and post-orthopedic or post-surgical patient care.
• Excellent customer service and communication skills.
• Experience with EPIC electronic charting system.
• Ability to work independently while collaborating with a multidisciplinary team.
