Key Responsibilities
- Provide skilled therapeutic services to patients in a skilled nursing facility (SNF) setting
- Assist with developing and implementing individualized treatment plans
- Monitor patient progress and adjust therapy as needed
- Collaborate with healthcare professionals to optimize patient outcomes
- Document patient assessments and treatment activities accurately
Required Qualifications
- Minimum of 1 year of professional SNF experience
- Valid license or certification as a Physical Therapy Assistant (if applicable)
- Strong knowledge of therapeutic techniques and patient care standards in SNF environment
- Excellent communication and teamwork skills
Preferred Experience / Skills
- Experience working with a diverse patient population
- Familiarity with electronic health records (EHR) systems
