Key Responsibilities
• Provide physical therapy services to residents to maximize their functional abilities and independence.
• Perform evaluative and therapeutic procedures under physician orders, observing all safety precautions and contraindications.
• Develop and implement individualized treatment plans, including setting functional goals and selecting appropriate modalities.
• Collaborate with residents, caregivers, and interdisciplinary team members to ensure comprehensive care.
• Monitor and document treatment progress, adjusting plans as needed to meet established goals.
• Educate residents and caregivers on the use of assistive devices and care techniques, documenting all teaching efforts.
• Ensure compliance with billing and reimbursement regulations, including Medicare and Medicaid.
• Supervise and mentor assistant-level staff when appropriate, ensuring adherence to professional standards.
• Maintain a clean and safe clinical environment, following infection control procedures.
• Stay current with professional development through memberships and continuing education activities.

Required Qualifications
• Bachelor's degree in Physical Therapy (Doctorate preferred).
• Valid and active Physical Therapist license in the state of practice.
• Basic Life Support (BLS) certification preferred.
• Prior experience in long-term care or skilled nursing facilities is a plus.
• Strong communication and interpersonal skills.
• Proficient in documentation and Microsoft Office software.

Preferred Experience / Skills
• Experience working in a skilled nursing or long-term care setting.
• Knowledge of Medicare, Medicaid, and private insurance documentation requirements.
• Demonstrated ability to work collaboratively within an interdisciplinary team.
• Excellent time management and organizational skills.
