Schedule / Job Type
- Full-time, on-site
- Shift: 7 days on / 7 days off, 10-hour shifts
- Days: Friday through Thursday, 12:00 PM – 10:30 PM
Key Responsibilities
- Provide skilled hospice nursing care to patients in their homes
- Manage patient admits, triage, phone consultations, and in-home RN visits
- Document care activities using EPIC and Home Care Home Base systems
- Collaborate with interdisciplinary teams to ensure quality patient care
- Travel within the service area, with minimal drive time and groupings to optimize efficiency
Required Qualifications
- Minimum 1 year of hospice nursing experience
- Experience with Home Care Home Base (HCHB) system required
- Valid and current RN license in Washington or a compact state
- Basic Life Support (BLS) certification
- Reliable transportation for home visits
Preferred Experience / Skills
- Experience with EPIC electronic medical records system
- Ability to manage multiple patient visits and coordinate care
- Familiarity with hospice or home health standards and protocols
