Key Responsibilities
- Provide skilled nursing care to patients in a Long-Term Care / Skilled Nursing Facility settings
- Administer medications and monitor patient health conditions
- Document patient care activities accurately and promptly
- Collaborate with healthcare team to develop and implement care plans
- Ensure compliance with facility policies and healthcare regulations
Required Qualifications
- Valid LVN or LPN license in the state of New Jersey
- Proven experience in a Long-Term Care or SNF environment
- Strong patient care and documentation skills
- Ability to work night shifts and adapt to the facility's schedule
Preferred Experience / Skills
- Prior experience working with elderly or chronically ill patients
- Knowledge of electronic health records systems
- Excellent communication and teamwork skills
