Key Responsibilities

• Provide occupational therapy services, including evaluations and treatments, to patients across multiple settings.
• Develop and implement individualized care plans in collaboration with COTAs.
• Serve patients in a Critical Access Hospital, outpatient therapy, home health, and long-term care settings.
• Contribute to patient recovery goals by applying evidence-based OT techniques.
• Document patient progress and adjust treatment plans as necessary.

Required Qualifications

• Valid Occupational Therapist license in the state of Colorado.
• Recent experience providing OT services in hospital, outpatient, or home health settings.
• Ability to assess patient needs and develop appropriate treatment plans.
• Strong communication and teamwork skills.
• Basic documentation and electronic health record (EHR) proficiency.

Preferred Experience / Skills

• Experience in hospital or critical access hospital settings.
• Knowledge of outpatient and home health therapy practices.
• Certification in specialized areas of occupational therapy (if applicable).
