Key Responsibilities

• Provide patient care and case management to home health, hospice, and palliative care patients.
• Conduct initial patient evaluations, develop and revise individualized plans of care.
• Deliver clinical services requiring advanced nursing skills and expertise.
• Coordinate patient services and communicate changes in patient conditions to physicians and care teams.
• Educate patients and families on nursing needs, preventive, and rehabilitative procedures.
• Document clinical notes and participate in ongoing in-service training programs.

Required Qualifications

• Valid Registered Nurse (RN) license.
• Experience in hospice or home health care preferred.
• Strong assessment and communication skills.
• Ability to collaborate effectively with multidisciplinary teams.
• Ability to work in a patient-centered environment with compassion and professionalism.

Preferred Experience / Skills

• Previous experience in hospice or palliative care.
• Knowledge of relevant nursing practices and procedures.
