Schedule / Job Type
- Full-time, temporary contract, outpatient/ambulatory care setting, 12-hour shifts, approximately 3 shifts per week
Key Responsibilities
- Assist registered nurses and licensed practitioners with patient assessments, health history collection, and admission processes.
- Document patient responses, evaluate and contribute to the care plan, and modify interventions collaboratively.
- Perform patient care and personal cares, ensuring safety and quality standards are maintained.
- Identify early signs of patient condition changes, implement nursing interventions, and communicate updates promptly.
- Collaborate with multidisciplinary team members to deliver patient-centered care and coordinate discharge planning, education, and counseling.
- Delegate appropriate nursing tasks to qualified team members and ensure documentation accuracy.
Required Qualifications
- Valid LPN license for the state of Minnesota.
- Basic nursing skills and familiarity with outpatient or ambulatory care procedures.
- Ability to document and communicate patient information clearly and accurately.
- Competency in patient assessment and the timely execution of care plans.
- Knowledge of infection control and safety protocols.
Preferred Experience / Skills
- Previous experience in outpatient, clinic, or ambulatory care settings.
- Strong communication and team collaboration skills.
