Job Title

Schedule / Job Type
• Full-time, Day shifts, 3 days per week, up to 12 hours per shift

Key Responsibilities
• Provide in-home occupational therapy services to patients receiving home care and hospice support
• Assess patients' needs and develop individualized treatment plans to promote independence and safety
• Collaborate with a multidisciplinary team to ensure comprehensive patient care
• Document patient progress and communicate with family members and healthcare providers as needed
• Travel to patients' homes utilizing personal transportation, maintaining reliable vehicle use

Required Qualifications
• Vermont licensure as an Occupational Therapist
• Valid driver’s license and reliable vehicle for travel
• Ability to work independently in a home environment

Preferred Experience / Skills
• Prior experience in a home care or hospice setting
• Strong interpersonal and communication skills
• Ability to work effectively as part of a multidisciplinary team
