Key Responsibilities

• Provide physical therapy assessments and develop individualized treatment plans
• Implement therapy interventions to improve patient mobility and function
• Monitor patient progress and adjust treatment plans accordingly
• Document patient care and progress notes in accordance with facility protocols
• Collaborate with healthcare team to ensure comprehensive patient care

Required Qualifications

• Valid state license or certification as a Physical Therapist
• Proven experience providing physical therapy services
• Strong knowledge of therapeutic techniques and evidence-based practices
• Excellent communication and patient care skills

Preferred Experience / Skills

• Prior experience in a healthcare or outpatient setting
• Ability to work independently and as part of a multidisciplinary team
