• Full-time, Days shifts, approximately 4 shifts per week
Key Responsibilities
• Evaluate and develop treatment plans for patients with injury or conditions affecting mobility or independence
• Provide specialized rehabilitation services including orthopedic, neurological, stroke, balance, dizziness, and chronic pain management
• Assist patients in regaining functional abilities and achieving community activity goals
• Document patient progress and communicate treatment outcomes to healthcare team
• Travel to multiple care points as needed to ensure comprehensive patient care
Required Qualifications
• Current unencumbered Oregon Physical Therapist license
• Healthcare Provider Level BLS certification
• Ability to travel to various care locations within the community
Preferred Experience / Skills
• Experience in orthopedic, neurological, or movement-based rehabilitation preferred
• Strong communication and patient care skills
• Ability to work independently and as part of a multidisciplinary team
