iDEAL HIRE is hiring a new Physical Therapist - Skilled Nursing / Rehab for our client, a Skilled Nursing and Rehabilitation facility.

Role Details

• Compensation: $50 - $65 per hour with a $2,500 sign on bonus
• Benefits: Medical, Dental, and Vision
• Duration: Needed ASAP - Current
• Location: Phoenix, AZ 85037

• Establish and administer comprehensive treatment plans with specific goals based on patient capacity and physician direction.
• Assess patient needs and modify care plans to restore function, improve mobility, and relieve pain.
• Collaborate with interdisciplinary teams to evaluate and achieve goals for each resident in a 54-bed skilled facility.
• Document patient progress and conditions meticulously in accordance with state and federal regulations using Net Health and PCC EMR systems.
• Educate patients and their families on appropriate therapy methods and post-discharge care.
• Oversee and direct the screening of residents to optimize overall functionality and clinical outcomes.
• Maintain a collaborative relationship with the nursing department within a modern, high-energy facility culture.

• Education: Bachelor’s, Master’s, or Doctorate Degree from an accredited Physical Therapy program.
• Licensure: Active Physical Therapist license in the state of Arizona.
• Certification: Current CPR certification required.
• Experience: Six months of experience in a long-term care or skilled nursing environment preferred.
• Skills: Proficiency with EMR systems (Net Health/PCC preferred) and the ability to work in a fast-paced, collaborative setting.
