Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility (SNF).
• Document patient progress and maintain accurate records.
• Collaborate with the healthcare team to develop and implement treatment plans.
• Ensure compliance with all regulatory standards and facility policies.
• Maintain productivity targets, achieving PTA productivity of 93-95%.
• Deliver therapy services during 8-hour shifts on a 5-day workweek.

Required Qualifications

• Valid licensure/certification as a Physical Therapy Assistant.
• Minimum of 1 year of professional SNF experience.
• Proven ability to meet productivity levels (PTA 93-95%).
• Strong communication and teamwork skills.
• Ability to work independently and adhere to treatment protocols.

Preferred Experience / Skills

• Experience working specifically in skilled nursing facilities.
• Knowledge of healthcare documentation and electronic medical records.
