Community Hospice - Case Manager (Registered Nurse)

Schedule / Job Type

Contract | Days | Monday–Friday, 8am–5pm with occasional weekend back-up and on-call

Key Responsibilities

• Manage an assigned caseload of hospice patients, leading the interdisciplinary team in delivering personalized care
• Provide compassionate clinical care to patients wherever they call home, including private residences, nursing homes, group homes, and assisted living facilities
• Educate patients and caregivers to help identify personal goals of care and build caregiver confidence
• Perform comprehensive assessments and apply critical thinking for symptom management across a broad range of diagnoses
• Serve as a generalist across disease processes with a specialization in end-of-life symptom management

Required Qualifications

• Active Registered Nurse (RN) license in good standing
• Strong clinical assessment and critical thinking skills
• Experience in hospice, palliative care, or a related clinical specialty
• Ability to work a Monday–Friday schedule with occasional weekend back-up and on-call availability

Preferred Experience / Skills

• Prior experience as a hospice case manager or primary RN managing an independent patient caseload
• Experience educating patients and family caregivers in a home or community-based care setting
• Familiarity with interdisciplinary team care models in a hospice environment
