Contract | Rotating Schedule | Onsite | Full-Time | Tuesday–Saturday or Sunday–Thursday (5-day work week with possible weekend day coverage)

Key Responsibilities

• Perform diagnostic assessments and develop individualized treatment plans for patients across SNF/inpatient and outpatient settings
• Provide professional physical therapy services for the full range of cases within a habilitation and rehabilitation program
• Implement and monitor treatment plans, adjusting as needed based on patient progress and clinical findings
• May provide all therapy services for a unit or facility as needed
• Utilize Meditech EMR system for documentation and patient records
• Maintain availability to work shifts, weekends, and holidays as required based on departmental operations

Required Qualifications

• Active State of Hawaii Physical Therapist license required by assignment start date
• Minimum 6 months of work experience in at least one of the following adult care settings: Acute, SNF, Rehabilitation Hospital, or Outpatient
• Current Basic Life Support (BLS) certification
• Ability to work a rotating schedule including weekends and holidays

Preferred Experience / Skills

• Experience working in a SNF or long-term care environment
• Familiarity with Meditech EMR documentation system
• Experience providing therapy services across both inpatient and outpatient settings
